Completed Physical Exam Forms Due PRIOR to GLAX Season Start
- All WHS Athletes must have a completed Physical Exam Form submitted to the Athletic Department prior to the season start to be eligible for participation. If your athlete was in a Fall or Winter sport, they are already covered for the current school year. If not, here is the language from the WHS Website:

- A student may not participate in interscholastic athletics (practice or competition) until the school has a physical examination form that attests to the student’s physical fitness.
- A physical examination taken/performed April 1 and thereafter is valid for the following two school years; a physical examination taken/performed before April 1 is only valid for the remainder of that school year and the following school year.
- The student may get a physical exam form from a coach or the activity director’s office, or at the Waunakee medical clinics.
- Complete the physical exam and return the card to the activity director’s office with all of the information properly filled in and signed by your parent or guardian.
- The status of the physical exam paperwork on file can in the Infinite Campus Parent Portal.
- Exams can be coordinated via your athlete’s regular physician, or Waunakee High School offers the following contacts for families who may need to schedule Physical Exams for sport eligibility prior to the season:

- If your athlete HAS had a physical exam within the eligible timeframe listed above, you simply need their physician to complete the Physical Exam Form for them and submit it, if you have not already.
